Notice of Change: All of our sample information forms have been revised. When filling out the new forms, please provide your e-mail address so we can notify you when your reports are ready. As of November 1, 2009, paper reports will no longer be mailed.

Only for return customers — First-time users must use one of the forms in the above list.

Go to PALS. Select the Grower Login link, and log in as yourself.

Click on Enter Your Samples Online. Fill in your information. Print copies to file and submit with your sample(s).

Note: You will not be able to fill out an online sample information form until you provide the Agronomic Division with a valid e-mail address. To do this, log in as yourself, then go to the Register Your E-mail Page and follow the instructions.
