Complaint for Violation of Notification Rule
The following regulation took effect in North Carolina on January 1, 2003:
02 NCAC 38 .0705 NOTIFICATION OF FORMER
SUPPLIER BEFORE DISCONNECTING
You may submit a complaint online that a company did not provide the required notification of a container disconnect.
By completing and submitting this online form to the NCDA&CS Standards Division, you are making a complaint that another propane company has disconnected your company's tank at one of your customer's location and that company has not provided written notification received by your company prior to the disconnect, in accordance with the above regulation. If you do not want to complete this online form, you may download a form to mail or fax.
If you submit a complaint online, please do not send a duplicate paper copy.
All fields marked with an asterisk (*) must be completed with significant information. Failure to provide significant information in every required field invalidates submission.
