Spay Neuter Program

You will NOT be reimbursed if the you fail to submit the following:

• Completed Request for Reimbursement, signed by the county or city manager and notarized
  
  
  • This must be submitted by the last day of the month following the end of the quarter.
    

• Itemized list of costs for reimbursement sought
  
  
  • If using an agreed upon "flat rate" through a veterinarian a flat rate break down should also be submitted. The same flat-rate breakdown may be used each quarter, unless the flat-rate changes.
  
  
  • You will NOT receive reimbursement for any charges other than those directly related to the cost of spay/neuter procedures owned by low-income individuals.
    

• A rule/guideline established by your county/city designating what proof a low-income person must submit to establish eligibility for your low-cost spay/neuter program
  
  
  • This is a one-time submission to the Spay/Neuter Office, unless there is a change to the rule/guideline.
    

• Annual Shelter Report
  
  
  • This is submitted yearly and spans a calendar year.
