Spay Neuter Program

You will NOT be reimbursed if you fail to submit the following:

  • Completed Request for Reimbursement form (available, here), signed by the county or city manager and notarized

    • This must be submitted by the last day of the month following the end of the quarter.
  • Itemized list of costs for reimbursement sought

    • If using an agreed upon "flat rate" through a veterinarian a flat rate break down should also be submitted. The same flat-rate breakdown may be used each quarter, unless the flat-rate changes. An example of a flat rate breakdown is available, here.
    • Examples of itemized lists are available, here.
    • You will NOT receive reimbursement for any charges other than those directly related to the cost of spay/neuter procedures owned by low-income individuals.
  • REVISED COMPONTENT! A rule/guideline established by your county/city designating what proof a low-income person must submit to establish eligibility for your low-cost spay/neuter program. THE DEFINITION OF LOW-INCOME HAS CHANGED TO AN INDIVIDUAL WHOSE ANNUAL HOUSEHOLD INCOME IS <100% OF THE FEDERAL POVERTY LIMIT OR WHO QUALIFIES FOR ONE OR MORE OF THE PROGRAMS OF PUBLIC ASSISTANCE ADMINISTERED BY THE DEPARTMENT OF HEALTH AND HUMAN SERVICES PURSUANT TO CHAPTER 108A. ALREADY PARTICIPATING COUNTIES MUST REWRITE RULES/GUIDELINES TO REFLECT THIS CHANGE! Please see the following links for more information:

  • NEW COMPONTENT! Provide the opportunity to participate in your program to all "local veterinarians". You must make this opportunity available all "local veterinarians" each year. Please see the following links for the definiton of "local veterinarian" as well as examples of ways to provide the opportunity:
  • NEW COMPONTENT! Submit proof that you have provided the opportunity to participate in your program to all "local veterinarians". You must submit this documentation with the first reimbursement request of each calendar year. Please see the following links for more information as well as examples of documentation that may be used:
  • Annual Shelter Report

    • This is submitted yearly and spans a calendar year.
