Death Reporting Form

Use this form to report the death of a farmed cervid. All dead cervid, 12 months of age or older, must be taken to a North Carolina Diagnostic Laboratory for Chronic Wasting Disease (CWD) testing. All dead cervid, regardless of age, must be taken for testing if exhibiting signs of Chronic Wasting Disease.

Once you have filled out this form, click the "Submit Form" button and your information will be submitted to NCDA&CS. A copy of this form will be emailed to you and should be kept with your records.

This form must be completed within 24 hours of discovery of death.
