Standards - Instructions for Renewal of Petroleum Device Technician License
You must submit the application form to renew your unlimited Petroleum Device Technician License each year. It is important that you take immediate action in order to have your registration renewed prior to December 31 of each year. The following steps should be followed in completing the renewal form for each applicant.
1. Fill in or correct (if necessary) your complete mailing address, including zip code.
2. Fill in or correct (if necessary) employment information, including company name, mailing address and zip code.
3. Write in the state, date (month, day, and year), and test number of the latest calibration for your test measure or prover in the space provided.
Note: Your five-gallon test measure(s) must have been tested within the last 12 months.
Large provers for terminal meters must have been tested within the last 36 months.
Dynamic small volume provers (SVPs) must have been tested within the last 12 months.
4. If your test measure or prover was not tested by North Carolina, enclose a copy of the test report from the state where it was tested. This information must be included before your license can be processed.
5. Sign the renewal form where the applicant's signature is requested.
6. Enclose with your renewal form a lead and wire seal that is embossed with your three (3) initials on one side and the last two digits of the year of the license, for example, 09, for 2009, on the other. Your application cannot be processed until we receive this seal. Therefore, we suggest that you immediately order a die (embossed seal) for your hand sealer from the stamp and die or petroleum equipment dealer of your choice.
