Please complete the above application in order to apply for a movement permit. A movement permit is required to transport cervid to or away from your facility. Cervid may not be moved until an approved movement permit is received by the licensee. Movement permit applications may take up to 5 business days to review, approve, and issue a movement permit so please plan accordingly. Applications can also be emailed to farmedcervid@ncagr.gov or faxed to 919-733-2277.
Please complete the above form after moving your cervid per an approved movement permit. This form is confirmation of cervid moved from your facility and their specific identification numbers. Additionally, you can complete the online submission form using the link at the top of this page. Paper applications can be emailed to farmedcervid@ncagr.gov or faxed to 919-733-2277.
Cervid Death and CWD Sample Submission
Please complete the above application and bring with you when submitting your cervid to the lab for Chronic Wasting Disease (CWD) testing. In addition, you must complete the Death Reporting Form by using link at the top of the page.
