Online Submission
This is an intuitive, step-by-step process that ensures that the correct client account is chosen for sample submission and that all sample-specific information is provided.
Manual Submission
- Complete the predictive or diagnostic sample submission form. Predictive samples (most common) are preseason samples taken to predict potentially harmful nematode populations in the crop to be planted. Diagnostic samples are taken during the growing season to diagnose suspected nematode problems in the current crop. Select "Molecular Diagnosis" to add this test to the standard assay.
- If submitting more than six samples, use the nematode assay continuation page to list additional samples.
- To get accurate results and recommendations, you must provide details of crop history.
- Give each sample a unique identifier (sample ID) made up of five letters and/or numbers that will help you remember which area the sample corresponds to ex: GOOD, BAD, COT1, COT2, BEANS or LAWN. Write this ID in the designated spaces on both the sample box and on the submission form.
How to Package and Ship
- Place the samples inside a shipper according to the order that they are listed on the nematode sample information form. NCDA&CS provides two sizes of shippers free of charge; the large shipper holds 16 boxes. The small shipper holds 12 boxes.
- Growers should never send soil nematode samples to the lab in padded mailing envelopes as the samples can shift, crumble, and get damaged.
- Lastly, place the nematode sample information form on the top of the nematode sample boxes before closing the shipper.
If you are sending less than what the shipper holds, add packing material to avoid shifting of samples and damage during transit.

- US Postal Service: 1040 Mail Service Center, Raleigh, NC 27699. Allow additional time for delivery.
By Courier: Many NCSU County Extension offices offer a courier service to deliver laboratory samples to the Agronomic Division. Contact your County Extension office to inquire if this service is available in your area.
In person: Samples may be dropped off at the Agronomic Services Division's loading dock Monday-Friday between the hours of 7: 00 a.m. - 4:00 p.m., except N.C. State holidays.
